    Welcome to unit CHCCCS031 Provide individualised support.

    This unit covers the skills and knowledge required to: 

    • Determine personal support requirements 
    • Provide support services 
    • Monitor support activities 
    • Complete reporting and documentation 

    This unit is designed for who provide support services to people in a range of community services contexts according to their individualised plan. 

    Learning may take place online, in a classroom, at a workplace or in a combination of these. 

    This unit will give you the skills and knowledge required to organise, provide and monitor personal support services according to the person’s support or care plan.

    Assessment Summary

    For unit CHCCCS031 Provide individualised support, you are required to complete the following tasks:

    Assessment title Assessment method
    Assessment Task 1: Personalised support – Quiz Quiz questions: short answer, multiple choice, multiple response, drag and drop and case studies.
    Assessment Task 2: Providing personalised support – Observation Direct observation in a workplace/simulation, role play, presentation.  

    Your assessment results

    An assessor will review your work to make sure it is satisfactory. You will be given a ‘Satisfactory’ or ‘Not satisfactory’ for each assessment task. On completion of the unit, you will be graded as ‘Competent’ or ‘Not yet competent’.

    Your teacher or assessor can provide specific guidance on how your grade will appear online and in your academic transcript.

    Your assessor will provide feedback and a copy of your work at the completion of your assessment. You may need to redo some of your responses or provide more information. Your assessor will explain how to do this.

    If you are not happy with your assessment result, discuss this with your assessor.

    Preparing for assessment

    Each assessment outline provides you with the following information:

    Make sure you understand what is required of you before undertaking and submitting your assessment, and that you understand your rights and responsibilities as a student.

    Discuss any specific requirements

    Ensure you have discussed with your assessor any reasons for special consideration, reasonable adjustment, or extension of time to complete your assessment.

    If you cannot undertake an assessment task as described, discuss this with your assessor. You may be asked to complete the assessment in another way, such as saying answers out loud for a written assessment.

    Assessment coversheet

    You may be required to complete and attach an assessment coversheet for each assessment task. This document usually outlines training provider policies and procedures for assessment, including details on plagiarism, resubmissions, extension of time and declaration that students meet all associated requirements.

    Speak with your assessor about whether an Assessment Coversheet is required, and if there is a specific template for you to use. If your training organisation requires a coversheet and does not have a preferred template, you may complete and attach the Assessment Coversheet template provided.

    File naming protocols

    You may be required to upload documents as part of your assessment submission. This may include an Assessment Coversheet, a copy of a Third Party Reports, images, audio or video files.

    It is recommended that you follow the file naming protocols outlined by your training provider. If you are not able to locate these, you may follow the instructions below.

    Include the elements below in the filename of each file you plan to submit for assessment:

    • A personal identifier. This may be a:
      • StudentID
      • Your name in the format:
        • Surname
        • First name initial
        • Second name initial
    • Unit code
    • Assessment number
    • Date in the format YYYYMMDD
    • You may use an underscore ‘_’ to separate items for clarity
    • Add a short descriptor if needed, such as ‘CS’ for Coversheet

    You may refer to the following example as a guide.

    For Assessment 3 of unit CHCCOM005, John A Smith needs to upload a coversheet, a Third Party Report, and a video demonstration.

    He names the files as follows:

    • SmithJA_CHCCOM005_AT3_20240917_CoverSheet
    • SmithJA_CHCCOM005_AT3_20240917_ThirdPartyReport
    • SmithJA_CHCCOM005_AT3_20240917_VideoDemo

    Alternatively, he could name these same files:

    • 1234567X_CHCCOM005_AT3_20240917_CS
    • 1234567X_CHCCOM005_AT3_20240917_Part1
    • 1234567X_CHCCOM005_AT3_20240917_Part2
